Severe acne induced by clascoterone: A case report

This case report describes a 23-year-old woman who developed a severe, paradoxical acne flare three weeks after starting topical clascoterone 1% cream. Despite the drug's intended role as an androgen receptor antagonist to reduce sebum and inflammation, the patient experienced painful cystic and pustular lesions and facial edema. Her recent discontinuation of oral contraceptives may have been a predisposing factor due to hormonal instability. The reaction was classified as a "probable" adverse drug reaction, and the patient showed significant improvement only after discontinuing clascoterone and switching to a regimen of benzoyl peroxide and adapalene
